• FRAUD involving LSC grants or other funds;
• WASTE of money or resources;
• ABUSE by LSC employees or Grantees; or
• VIOLATIONS of laws or LSC regulations

UPON REQUEST YOUR IDENTITY WILL BE KEPT CONFIDENTIAL
REPORTS MAY BE MADE ANONYMOUSLY
