Appointment Success

Congratulations! 

Your appointment has been reserved.  You should receive an email confirmation of your appointment shortly.  Below is some information you should be aware of:

• We have several appointments each day and we may need some leeway.  Please allow 10-15 minutes on both ends of your scheduled time for us to call you;
• Make sure you are at the phone during your scheduled time;
• Make sure that you have enough time to finish the screening and perhaps talk to an advocate;
• We will only call you once.  If we are unable to get a hold of you, we will not try again and it will be your responsibility to either return to our website to chat online with us or to call us. 

If you discover that you are unable to be available, please contact ULS by clicking here as soon as possible to let us know.  We  can then make that appointment available to someone else.
