Clinic Forms

Hi all,

You will notice that there is now a "clinics" practice area.  I want to upload all the clinic materials there so they will be accessible to everyone.  Unfortunately, most of the clnic forms are in WP and in pieces.  If you have any packets that you use regularly at the clinic, will you please scan them into PDF and send them to me so we can have a complete repository of forms for use at clinics.  We will have to deal with Word conversion when we decide we need to updat them. Thanks in advance!
